Program Description: The REACH program provides case management services for HIV positive individuals, helping client’s access medical and supportive services, and coordinating client care. Case managers serve as client advocates and assist in the development of a personalized plan to address client needs and meet their identified goals.  Transportation is also provided for qualified clients via County van or bus passes to assure that they access necessary medical and mental health services.
