Telecare STEPS (Striving Towards Enhanced Partnerships) is a full service partnership program contracted with the Orange County Health Care Agency that serves 105 adults with persistent mental illnesses. Eighty slots are available for members coming out of residential or locked facilities and 25 are available for members who are referred by the Mental Health Court. The program is committed to connecting with each individual’s core self and awakening their desire to embark on a journey of recovery. Services are “wrapped around” clients to help them gain the skills and confidence to live successfully in the community. STEPS offers outreach and engagement, intensive case management, 24/7 availability, housing and medication support. They also offer dual diagnosis treatment, vocational and educational services and linkage to financial benefits.

STEPS utilize data and outcomes surveys to improve the quality of service we provide our members. We are able to tailor the levels of services to each individual needs by tracking outcomes in five key areas: education, employment, residential, hospitalizations and incarcerations. By tracking their progress and providing each member with the tools to help them succeed, wecan work closely together toward their individual recovery goals. Our focus is on increasing educational, employment and independent living goals for each individual member, as well as lowering rates of incarcerations and psychiatric hospitalizations.
