Child Health and Disability Prevention Program (CHDP)
Maternal, Child and Adolescent Health
The Child Health and Disability Prevention (CHDP) Program is a health promotion and disease prevention program aimed at providing services to children and adolescents.
Every local health department in the State of California has a Child Health and Disability Prevention Program. The County of Orange Health Care Agency, Maternal, Child & Adolescent Health Program is responsible for provider approval, outreach to families with children, community education, case coordination, and implementation of the state and federal regulations pertaining to the CHDP Program.
To become a CHDP certified provider through the County of Orange CHDP program, you must fulfill the following requirements:

Participate in the Vaccine for Children (VFC) Program

Documentation of “passed” site review

Completion of the CHDP Program application

Complete an on-site CHDP Program medical practitioner
and office staff training
