Provides consultation to Family Resources Center Health Access teams and home visitation to families referred to the FRC's. The Health Access goal is to reach medically and socially vulnerable children with accessible, culturally sensitive services that build on family strengths and improve overall health and well being of children served. The Public Health Nurse and the Promotoras (health promoters) team up to provide in-home assessments, interventions, and case management, and collaborate to organize community classes and health promotion events.
