The district model provides an opportunity to address the unique needs of a specific community through collaborative efforts to work closely with community members to assess and address community, family and individual health needs. Working on-site at a community agency, the PHNs utilize their skills and community resources and site supporting programs to prevent problems and provide access to care. PHNs participate in activities including home visitation, health education classes, collaborative activities and outreach, utilizing the center as a "home base". The PHN maintains office hours at the site to meet with community clients to provide case management, referrals to appropriate community resources and assist clients with establishing a medical home.
