Program services are free of charge. SHOPP is a voluntary program and clients may choose to start and stop participation at any time.
Eligibility for SHOPP Services
In order to be eligible for SHOPP services a person must:
Be age 45 or older
Have an unmet health care need An unmet health care need exists if a person has a health problem or potential health problem and is unable or unwilling to get the problem addressed
Confidentiality
All personal and medical information is confidential and will not be released except with written permission or by court order.
