The Medically High Risk Newborns Program (MHRN) provides Public
Health Nursing home visitation services to medically fragile
infants until they reach 24 months of age. Nursing interventions
include case management, parent education, counseling services
to children and families who have medical and psychosocial risk
factors which may impede the child’s access to health services
and family self-sufficiency.
Services Include:
Physical, developmental and nutritional assessments
Safety assessments
Parent/child interactions
Problem identification
Health teaching
Case management and follow-up
Referrals to community resources
Program Description:
State funded program
Eligible high risk infants from birth to 24 months residing in
Orange County
High Risk Criteria:
Graduate of a Bridges Neonatal Intensive Care Unit (NICU)
Born or discharged from a Bridges Hospital
Enrollment must take place within 90 days of discharge
Be at risk for developmental delays based on one or more of the
medical eligibility criteria
The Goals:
To prevent or minimize developmental disabilities or delays
To provide, decrease and coordinate a delivery system to reduce
the incidence of infant mortality and the impact of
potentially disabling conditions
