Program for Assertive Community Treatment (CYBH PACT)

Program Description: PACT (Program for Assertive Community Treatment) is a field-based program for transitional age youth and young adults ages 14-21 living with a chronic and persistent mental illness, who may have a co-occurring substance use disorder, and who are experiencing significant impairments in their daily functioning. Outpatient services include the following intensive mental health services: assessment, individual, family and group therapy, substance abuse treatment, medication support, crisis intervention, extensive case management, and advocacy.

Serves: Orange County youth between the ages of 14-21 years old who have at least two psychiatric hospitalizations or one psychiatric hospitalization for at least 10 days and/or incarcerations, and are referred by their Children and Youth Behavioral Health (CYBH) clinician.

Hours of Operation: 8 a.m.– 5 p.m., Monday–Friday

 

Youthful Offender Wraparound (YOW)

Program Description: The Waymakers' Youthful Offender Wraparound (YOW) provides a Full Service Partnership program for Serious Emotional Disturbance youth and young adults who have either received behavioral health services while incarcerated or require these services while on current probation.

Serving: Children and Transitional Age Youth (TAY), Age 12–26

Language(s): English and Spanish

Hours of Operation: 8 a.m.–8 p.m., Monday–Friday; 24-hours a day, YOW provides crisis services for all enrolled clients 24-hours a days / 7-days a week, as needed.

Collaborative Courts

Program Description: The Collaborative Court full service partnership was designed to meet the needs of unserved and underserved youth who have come under the prevue of the Juvenile Court. It provides services to clients and families in coordination with Children and Youth Behavioral Health (CYBH), Probation, Social Services, the various attorneys representing the client, family, the County and the Juvenile Court. It serves consumers, dependents and/or wards of the court who have serious emotional disturbances in addition to truancy and/or substance abuse related-issues. The FSP provides culturally competent, in-home and community-based behavioral health services to clients and their families that address client and family needs across all life domains and work to improve client self-efficacy and social competence.

Serving: Children and youth through age 12 to 26 who have serious emotional disturbances or severe mental illness referred by the Orange County Juvenile Court system.

Hours of Operation: 8 a.m.– 5 p.m., Monday–Friday; 24-hours a day, Collaborative Courts FSP provides crisis services for all enrolled clients 24-hours a day / 7-days a week, as needed

Project FOCUS

Program Description: Utilizing a “whatever it takes” community-based and client-centered approach where individualized care plans are developed for individuals and families, the Full Service Partnership Wraparound program of OCAPICA provides mental health services and intensive case management services. These services include support and assistance in obtaining benefits for low income families, health insurance, housing placements, parent education and support, tutoring, mentoring, youth recreation and leadership development. 

Serving: Children, Youth and Transitional Age Youth, Age 0 to 26 who have serious emotional disturbances or severe mental illness with one or more of the following: homeless or at risk of homelessness, history of psychiatric hospitalizations, first psychotic episode, in danger of failing school, uninsured exiting Social Services Agency or probation system, unserved or underserved due to linguistic or cultural isolation, with special needs and/or co-occurring disorders, with parents who have serious mental illness.

Hours of Operation: 9 a.m.–5:30 p.m., Monday–Friday. Project Focus provides crisis services for all enrolled clients 24-hours a day/7-days a week, as needed.

Project RENEW

Program Description: Project RENEW (Reaching Everyone Needing Effective Wrap) by Pathways of California serves Orange County children from birth to 18 years of age with serious emotional disturbance (SED)/serious mental illness (SMI), along with their families. The program focuses on children and their families who have been unserved or underserved, are and who may be homeless or at risk of homelessness. While the child is the identified participant, Project RENEW uses a Wraparound model, providing a team of individuals offering both formal and informal support to the entire family.

Serving: Children and youth, Aage 0-18, who have serious emotional disturbances or severe mental illness with one or more of the following: homeless or at risk of homelessness, history of psychiatric hospitalizations, first psychotic episode, in danger of failing school, uninsured exiting Social Services Agency or probation system, unserved or underserved due to linguistic or cultural isolation, with special needs and/or co-occurring disorders, with parents who have serious mental illness.

Hours of Operation: 9 a.m.–5 p.m. with 24-hours a day. Renew provides crisis services for all enrolled clients 24-hours a day/ 7-days a week, as needed.

STAY

Program Description: Support Transitional Age Youth (STAY) of Pathways of California is a recovery based program for serious emotional disturbance (SED) and serious mentally ill (SMI) transition age youth (TAY), ages 16 through 25, who would benefit from increased integration into the community and support to assist in the development of the skills essential to becoming a self-sufficient adult. Founded with a vision that all TAY have hope, are empowered, can be responsible for themselves and be engaged in meaningful adult roles, the program supports participants in discovering and deciding what they want for themselves and how to develop the strengths, abilities and skills applicable to many aspects of their adult lives. Serves: Transitional Age Youth, Age 16-25, who are unserved or under-served, have experienced several psychiatric emergencies, may be exiting the Social Services system and/or are homeless or at risk of homelessness
