Immunization Information for Schools and Child Care Centers

• Pertussis vaccine (Tdap) - Required for all 7th-12th graders in the 2011-2012 school year and for all incoming 7th graders for 2012-13 school year and beyond.  Visit Shots for School for Pertussis Information Resources 
• California School and Child Care Immunization Resources
• California School Immunization Record (CSIR) or "blue card" - School and childcare personnel are required to complete and update the blue card for each child based on the child's immunization record.
