Appointment Information

To schedule an appointment for a medical marijuana card, call 714-480-6707 or email MMIC@ochca.com . Available times are from 8:00 a.m. – 12:00 p.m. and 1:00 p.m. – 4:00 p.m., Monday through Friday.

Appointments are required; applications will not be accepted on a walk-in basis.

You must obtain the following before coming to apply for a card:

  1. A written recommendation from your physician (ensure that there is a copy in your medical file).
  2. A Medical Marijuana Program Application/Renewal form (English/Spanish).
  3. Call to schedule your appointment at 714-480-6707 or email MMIC@ochca.com .
  4. Verify you have all of the items listed on the “Application Appointment Checklist.”

  1. The original written recommendation from your physician on official letterhead, prescription pad, or Written Documentation of Patient's Medical Record (English/Spanish).
  2. A completed original MMIC Application/Renewal Form (English/Spanish).
  3. A government-issued photo identification (i.e., California State Driver's License, a California State ID Card, United States Passport, Veteran's Administration ID Card).
  4. Proof that you live in Orange County.  For example, current utility bill, California motor vehicle registration with your current address in Orange County, or mortgage payment receipt. P.O. Box address is not acceptable for address verification.
  5. Fees (Non Medi-Cal: $84.00; qualified Medi-Cal participants will receive a 50% reduction in fee: $42.00). Payments can be made by cash or money order. If you choose to pay by cash, exact change is appreciated. Application fees are non-refundable.

  1. It can take up to 35 days to receive your MMIC for a complete application.  In order to guarantee delivery, you will be contacted to pick-up your ID card when it is available.
  2. The physician's license will be verified with the Medical Board of California or the Osteopathic Medical Board of California.

NOTE:  The physician's recommendation will be verified with the physician's office.
