Adolescent Services (Ages 12-17)

Transitional Age Youth/Program for Assertive Community Treatment (TAY PACT) 

Provides outpatient services including assessment, linkage, individual and group therapy, extensive case management, advocacy, medication support, substance abuse treatment, and a variety of recovery services for individuals from age 18 to 26 who are living with severe and persistent mental illness and who are experiencing multiple significant impairments in their community functioning. 


Youth under 21 years of age may receive additional medically necessary services under Early and Periodic Screening, Diagnosis, and Treatment (EPSDT). EPSDT services include screening, vision, dental, hearing and all other medically necessary mandatory and optional services listed in federal law 42 U.S.C. 1396d(a) to correct or ameliorate defects and physical and mental illnesses and conditions identified in an EPSDT screening whether or not the services are covered for adults.
