COVID Testing Kits - Resource Request Update - December 9, 2024

Eligible California organizations are able to obtain COVID-19 at-home test kits through the Health Partner Ordering Portal (HPOP) until the end of 2025, or until federal testing resources are no longer available.  Eligible California organizations are entities that provide resources, services and/or care to the uninsured/underinsured and/or those over the age of 65, including:

  • Community based organizations (CBOs) that provide services and resources to the uninsured/underinsured, or those over the age of 65, including shelters, food banks, and faith-based organizations with food/clothing pantries and/or community outreach programs.
  • Licensed care facilities-including skilled nursing, DSS licensed residential care for the elderly, DSS licensed adult residential facilities, DSS licensed adult day care.
  • Community health clinics, libraries, tribal entities, and farmworker organizations.
  • Government entities (including Fire, EMS, Agricultural Commissioner, etc.)

To request access to HPOP, please register here before January 15, 2025: California Application for Federal COVID-19 Testing Resources.  After January 15, 2025, please contact the Orange County Health Care Agency Public Health Division at PHtestKitRequest@ochca.com to obtain current information on COVID-19 at-home test kit ordering.

COVID-19 at-home tests should be prioritized for those who are uninsured, underinsured, or have poor access to medical care or who are at high-risk for severe disease.

COVID-19 tests are provided by the California Department of Public Health through a partnership with the U.S. Health and Human Services (HHS) Administration for Strategic Preparedness and Response (ASPR) and any questions should be directed to the Orange County Health Care Agency Public Health Division at PHTestKitRequest@ochca.com


Facility/MHOAC Situation Report Instructions    
The Facility/MHOAC Situation Report should be completed by healthcare organizations to report operational status during large emergencies, disasters or when there is a situation or event that may disrupt patient care.

Resource Request Instructions

A resource request should only be completed when the organization has exhausted all resources such as internal supply chains, sister or affiliated facilities, vendors, third party suppliers and any other entity that the facility may be able to obtain the needed resources from.

When submitting a resource request, you must also fill out a Facility/MHOAC Situation Status report.
