The Community and Nursing Services Division (CNSD) provides preventative health and supportive services in the home and community, embracing principles of public health, nursing, and social and behavioral health.
Using a multi-disciplinary approach, we deliver health education, screenings, assessments, referrals, and connections to health and wellness resources to promote optimal health outcomes. Our team works collaboratively with families, caregivers, and community partners to identify needs early, reduce barriers to care, and support individuals in achieving long-term wellness. Through culturally responsive practices and evidence-based interventions, we strive to strengthen community health, enhance quality of life, and empower residents to make informed decisions about their health.
