The Community and Nursing Services Division (CNSD) provides preventative health and supportive services in the home and community, embracing principles of public health, nursing, and social and behavioral health.
Using a multi-disciplinary approach, we provide health education, screenings, assessments, referrals and connections to health and wellness services to promote optimal health outcomes.
