We are a Public Health Nurse home‑visitation program supporting prenatal and parenting women, especially those navigating substance use disorders or living with HIV. Our nurses build trusting relationships, offering compassionate care, education, and advocacy. We also provide specialized nurse visits for Medically High‑Risk Newborns (MHRN), ensuring our most vulnerable infants receive the attentive, skilled support they need to thrive.

- To assist with health, emotional, alcohol, or substance use concerns
- To provide health education for mothers and babies
- To educate about growth and developmental concerns
- To coordinate care and case management for mothers and Medically High-Risk Newborns
- To access community services
